What You’ll Need for Your Office Visit

WE ACCEPT:

TCRS wants to make every office visit a hassle-free and pleasant experience. Before your visit, please download the MyCarePlan app to complete your registration and medical history. This will significantly reduce the check-in process. When you come to the TCRS office have the following information readily available:

  • Most recent insurance card
  • Driver’s license
  • Written list of questions you may have
  • List of current medications
  • Relevant test results, if any
  • Referral from your primary care physician, if applicable
  • X-rays from referring physician

Plan to arrive 15 minutes prior to your appointment and have a means of covering your financial portion as determined by your insurance plan. If you will be paying by credit card, we accept Visa, MasterCard, and Discover.

If your insurance requires a referral, please contact your primary care provider for the written referral. Your primary care physician may send the referral directly to our office. To expedite your visit, it is best if we receive the referral prior to the day of your appointment.

TCRS also offers the use of SmartHealth PayCard. You can apply online by clicking here.

SmartHealth PayCard is the new way to pay for:

  • Deductibles and Co-pays
  • Insurance Premiums
  • Elective Procedures
  • Unexpected Medical Expenses
  • Deep Discount Prescriptions
  • All Day-to-Day Medical Expenses

Billing Procedures

For services provided during office visits, payment is required at time of service. Co-pays and/or co-insurance for those patients with Medicare or one of our contracted managed care plans are also required at the time of your visit.

Additional charges may be generated for procedures and tests. TCRS will bill your insurance carrier for expected payment. You will be responsible for any deductible, co-pay amount, or co-insurance.

If it is determined that surgery is necessary, we will coordinate payment arrangements for any procedures your insurance company determines is your financial responsibility. Payment plans can be arranged prior to surgery if necessary. Did you know you can now pay your bill online?
